Body Weight Changes of Non-Insulin Dependent Diabetic patients in Korea

Abstract
Background:
@EN It is well known that Korean NIDDM patients, in contrast to Caucasians, are mainly non-obese and have experienced severe weight loss during the course of disease and many patients require insulin treatment, But there are little informations
about
the body weight changes of Korean NIDDM patients, the important point of difference from Caucasians, and the clinical characteristics according to the pattern of weight changes. So, we studied the patterns of the changes in body weights and body
mass
indices (BMI) of NIDDM patients before and after the diagnosis and the differences in clinical characteristics according to the patterns of changes in body weight.
@ES Methods:
@EN The data on the sequential body weight at twenties, maximal obesity, diagnosis and the amount and duration of weight changes before and after the diagnosis were obtained retrospectively from 256 Korean NIDDM patients without debilitating
disease or
drug history by careful history taking and questionnaires. The weight and height at recruitment were measured. We investigated the annual rate of weight gain from twendes to the time at maximal weight and the annual rate of weight loss from time
at
maximal weight to the tie at recruitment. Obese group was defined by BMI greater than 27§¸/§³ in males and 25§¸/§³ in females.
@ES Results:
@EN The non-obese NIDDM patients were 76.7% at recruitment, but about half of patients were obese at the time of maximal weight. One third of non-obese patients at diagnosis or recruitment had been obese before diagnosis. The cases with weight
loss
greater than 5§¸within 5 years before and after the diagnosis were 37%. Obese patients at diagnosis or recruitment were female predominant and insulin treatment was more frequent in non-obese NIDDM patients. Patients with weight loss(>5§¸ within
1
year)
received insulin treatment more frequently than those without any significant weight loss. Patients remaining non-obese from the time at maximal weight had lower level of fasting serum C-peptide and higher percentage of insulin treatment than
those
remaining obese. In multiple logistic regression analysis, the longer the duration of diabetes or the lower the BMI at diagnosis or the larger the amount of weight loss, the more patients receive insulin treatment.
@ES Conclusion:
@EN About one third of non-obese NIDDM patients in Korea were obese before diagnosis and have changed to non-obese through severe weight loss before and after the diagnosis. In respect that many of patients with not only low BMI at diagnosis but
also
severe weight loss before and after the diagnosis undergo insulin treatment, it is suggested that the part of cases with such clinical characteristics may be slowly progressive IDDM patients. The prospective study on the clinical charmcteristics
of
such patients in needed.
